The Pathways To Care Program is your road to access free Medications and Health care for people living with HIV/AIDS in Erie and Niagara County! We have dedicated Health Access Specialist on staff that are available to:

Screen individuals for healthcare options and determine eligibility for the following:

• The HIV Uninsured Care Programs
• Medicaid
• Medicare
• NYS Bridge Plan
• Direct Pay Plans
• Employer related Health Insurances
• To learn more about the services we have to offer click here.

Assess individuals for other community resources by providing linkage and referrals to:

• Health care providers
• Case management services
• Mental Health
• Substance Abuse programs

Educate providers and the community regarding:

• The HIV Uninsured Care Programs and other Health Care Programs
• The enrollment process
• And provide updates
