Screen individuals for healthcare options and determine eligibility for the following:
- The HIV Uninsured Care Programs
- Medicaid
- Medicare
- NYS Bridge Plan
- Direct Pay Plans
- Employer related Health Insurances

Assess individuals for other community resources by providing linkage and referrals to:
- Health care providers

- Substance Abuse programs
